If anyone's interested, I scanned the info sheet for the window. There's "fill in the blanks" for your info. (I think "Back in Black" did this a few years ago, and I thought it was very cool)

Gives your sheet a pro-look.

(Tip: leave your color printer set to print in color. If you change it to "print colors as black" it gets blotchy)
